1. Please call 913-469-1225 to schedule an appointment.

2. To make your first visit to the office as easy and efficient as possible,
our intake forms can be printed off and completed before your arrival.
If you don't have access to a printer, you can fill out the paperwork at the clinic before your first visit.
The intake forms include:
:Registration- Consent for treatment as well as Insurance Information (Required)
:Intake Form- Medical History and Family History (Required)
:Medical Records Release- Obtain records from previous clinics (Optional)
:Third Party Communication- Allow wellbody to discuss with teacher/coach (Optional)

Are you coming to the clinic for a sports physical for school?
Print out the form below and FILL OUT THE FIRST PAGE ONLY
Bring all the forms with you for your scheduled visit.

3. When bringing your child to their first visit to wellbody
please bring:
: THE COMPLETED INTAKE FORMS (above)
: CHILD'S health insurance card
: COPAY
: YOUR driver's license
(and your child's drivers license if they have one)
At your first visit to the office you can request to fill out a
consent for any follow up visits:
: that your child may be seen with another adult (such as a nanny, grandparent, or friend)
: that your child can come alone (if over 16 years old)
: that your child's sport's photograph can be used in our gallery or on our web site and newsletters
4. When bringing your child to any subsequent visits to wellbody please bring:
: CHILD'S health insurance card
: COPAY
: ANY BALANCE DUE ON YOUR ACCOUNT- Partial or full payment is necessary before visit
:YOUR driver's license (and your child's drivers license if they have one
